OPS-NHR-BUG-001 — User dashboard exposed agent claim control

Area: User-facing coordination dashboard
Summary: The dashboard exposed Claim, Submit review, and todo-creation controls intended only for Codex/Claude coordination.
Steps to reproduce: Open the dashboard, find an open plan item, and select Claim.
Expected result: The user sees project progress and questions requiring a response; internal ownership controls remain hidden.
Actual result: The item was claimed by the user session without clear feedback explaining what happened.
Impact: An accidental user-owned claim temporarily appeared on FTR-NHR-007. No project content was lost and no external system changed.
Root cause: The first client combined the agent task queue with the user-facing project-status view.
Resolution: Removed user-facing mutation controls, retained backend agent coordination, added read-only status pages, and released the accidental claim.
Verification: Seven proof tests pass, the event chain verifies, and the dashboard exposes no claim controls.
